Our Midwives provide care for mothers before and after delivery and care for the baby for their first ten days of life.
Community Psychiatric Nurses
These are based at Brierley Hill Health Social Care Centre – 01384 321484.
Patients who need home nursing are referred to the District Nursing Team by their GP or by the hospital.
If you have an illness or incapacity that means that you need nursing care in your own home one of our District Nurses will visit you. Patients who are likely to benefit from this service include the housebound, the elderly, people with a terminal illness, and those who have recently been discharged from hospital. District Nurses provide wound care, palliative care, continence advice, health promotion advice and advice on co-ordinating care packages.
If you move permanently into a care home locally, the District Nurse may be able to continue your care. Your GP may also be willing to continue to care for you, or a number of GPs may provide services for the residents of local care homes.
Practice Based Pharmacists
Hardip and Rita are Practice Based Pharmacists who work within the practice reviewing and advising on medication. Rita is in the practice Thursdays, while Hardip is available on Mondays.
Kuldip, Mohammed and Karen work across the PCN practices reviewing and advising on medication.
First Contact Physiotherapists (FCP)
Tom is in the practice all day on Mondays and James is with us on Wednesday afternoons. They are able to assess patients with specific types of pain e.g. back, shoulder.
Health Visiting Team
These are based at Brierley Hill Health Social Care Centre – 01384 321489
Our Health Visiting Team provide care for children from 10 days old until they start school.
