Cranstoun Social Prescribing Service

Cranstoun Children and young people’s social prescribing service will provide personalised non-medical, easy to access interventions to Children and Young People (CYP) aged 11–25 years with a GP within one of the 6 Primary Care Networks (PCNS) within Dudley. 

Cranstoun Children and young people’s social prescribing service will: 

  • Improve health and wellbeing through holistic, person-centred interventions. 
  • Address social determinants of health: isolation, low-level mental health needs, lifestyle challenges. 
  • Empower CYP and their families to take control of their health via goal-setting and personalised plans. 
  • Reduce demand on clinical services by offering community-based solutions. 
  • Work in the best interest and safeguarding of children and young people at all times.

  • Professional referrals from CYP CAMHS Single Point of Access
