Help Us Help You – What to include on the ‘Get Help for Any Health Problem’ form

When using the ‘Get help for any health problem’ form, it’s essential to provide as much information as possible. This helps the Triage GP understand your issue and safely decide the most appropriate care – without needing to message back and forth for more details.

🔹 Describe your symptoms clearly

Include:

  • When they started
  • How they feel (e.g. sharp, dull, constant, on/off)
  • If anything makes them better or worse
  • Any over-the-counter treatment tried

🔹 Include your availability

Please let us know when you can attend an appointment. Many times, appointments are offered but not suitable – this causes delays for you and others.

🔹 Check Pharmacy First

Before submitting a request, consider if the issue can be managed by a Pharmacist through the Pharmacy First service. Many common conditions can be treated quickly and easily at your local pharmacy, saving you time and easing GP workload.
