We can only accept new patients who live in our practice area (please see our practice area map below or alternatively please ask at reception for further details). You can register by completing a Registration Form, available here or which can be obtained from the practice. You will need to bring the completed form to reception along with your medical card if you have it. We will also require proof of identity and address in the form of a current driving licence or current passport and household utility bill, correspondence to you at this address etc. The information on the medical card will speed up the process of getting your records from your previous practice. Once you have been accepted as a patient your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
When you register, we will ask you to make an appointment to see either the practice nurse or healthcare assistant for a new patient health check. This will make sure that our records are accurate and all routine checks to promote your health and wellbeing (such as blood pressure/smear tests/mammograms) are up-to-date. New patients will be asked to complete our Health Check, Ethnicity and Alcohol Screening questionnaires, available here.
Patients that will be registered with the Practice but can choose a preferred doctor if they wish to do so. This choice will be offered during the health check process.
If you are only in the area for a short time (more than 24 hours), you may register with the surgery as a temporary patient for up to 3 months. If you are staying for a longer period of time, you will be required to register with the surgery in the usual way.
Persons not registered with us who require immediately necessary treatment owing to an accident or emergency will also be offered treatment for their condition. If the level of treatment required is outside the scope of the health professional available at the surgery the patient will be sent to hospital.
You will need to complete a Temporary Resident Form, which is available for download here or at reception. Once completed please return to reception. We will also require proof of identity and address in the form of a current driving licence or current passport and household utility bill, correspondence to you at this address etc. To make an appointment (please inform the receptionist that you are booking an appointment as a Temporary patient). Patients between the ages of 16 and 75 who have not seen a Doctor or Nurse for any reason during the last three years are invited to attend for a health check to enable us to update your records.
If you are over 75 and we have not seen you within the last year, then please arrange an appointment with either the Doctor or Nurse, this will help us to offer you the most appropriate help and advice regarding your health needs and medication. If you are unable to attend the surgery our nursing sister for the elderly will be pleased to visit you at home and arrange for a doctor to visit if necessary.
